Do anyone have filled examples of Is and Is Not Analysis ?

Can any one share filled examples of Is and Is Not Root Cause analysis or Kepner Tregoe analysis ? Examples from Manufacturing industry would be great.

The In/Out Frame was the method preferred by AlliedSignal 6-sigma as an IS/IS-NOT display method. What is fully IN the frame = IS a major contributor to the issue, what is on the frame edge is a suspected contributor, and what is outside the frame was considered, but was found that it 'IS NOT' a contributor to the issue.

This can lead to an assessment of the major interaction potential between the identified contributors. Both of these are attached as examples from a completed Design Enhancement project though some information has been redacted.

Thanks Sturmkind but I was looking for an Is and Is Not analysis which has various Dimensions like WHAT, WHERE, WHEN and SIZE and against each of these are IS questions and IS NOT question.

For example - the WHAT will have the following IS questions -

What specific item/object/unit(s) has the defect?
What specifically is the defect?

and the IS NOT will have the corresponding questions -

What similar item could logically have the defect, but does not?
What other defect(s) could logically exist, but do not?

Once you have both answers, there is another column to write the Distinctions and Changes.

I have attached a template.

There was a good non-analytical 5W, (5 Whys), post. It seems to add more uncertainty then get at root causes.
I feel a slam-dunk post coming behind mine!

I've been using the Kepner Tregoe approach for about 20 years - great process - would really recommend it - the is/is not question approach is particularly valuable to avoid jumping to conclusions without taking all relevent information into account
