Summarise your issues with all the information and look for any specific correlation w.r.t. customerwise / productwise / locationwise etc., Do 4M analysis for cause identification. You will chase your concerns.

we can check the end of pipe to analyze the outcomes ! it's bad method & too late also ; where's the competency of each process owner !! as example where's the system to prevent sending products to wrong address/location ' where the prevention the errors to occur !! is 'it really three's company system based on risk thinking for the prevention !!??
