I am new to quality in the software industry. I have so many times about FMEA but I am unaware about it. Please someone Help me....to know... what is FMEA and how is it performed in Software industry???

If any basic presentation materials are there...please share me so that I can get to know this topic effectively...
Firstly to get very basic knowledge... what to read and how to go through this topic ....

Hi, I am looking for an example of a (completed) DFMEA, related to (Micro) electronics. I am not an electronic engineer from backgrouwn but have to deliver a training for this kind of audience. One of the requests coming from the organization was to create a fictional example to work with the class to complete a DFMEA in the first place and to continue this into a PFMEA. The business is running in communication technology, sensor systems, safety systems and so on.

I already have scanned the internet (Elsmar, fmea-fmeca.com and fmeainfocentre.com). I would appreciate your replies.

Regarding Classification colum do you use also OS(Opearator Savety) and HI (High Impact ) Characteristics?
In this attached form the formula for CLASS concerns only CC and SC (defined in FAP 03-111

1) As per fourth edition there will be no limit to the RPN (e.g some comapnies take action above 100 RPN ONLY ) is it true???

2) AND IF S*O=36 even then action can be taken

WAITING FOR THE ANSWER,THANKS IN ADVANCE

This 100 RPN threshold was never a requirement in any of the versions of the FMEA manuals. That is something companies put onto themselves. As an auditor, I found FMEA procedures stating that if an RPN value dropped below 100, no action needed to be taken. Obviously, all the RPN values were below 100. :mg:

An organization must decide on actions taken.

The FMEA manual has recommendations as far as taking action is concerned.

Look it up under "Recommended action."

Yes, on first sight, it looks a little strange...but it's not a mistake. Here is why.. Occurrence rankings are influenced by the prevention controls provided that they are formally integrated in the process. For example, Poka Yoke's (which make a process more robust/error-proof) have a direct impact on the Occurrence rankings. That's why the Root Cause and Prevention columns are placed next to each other. You can still use the more traditional FMEA form if you wish. The 4th Edition manual offers various layouts. Remember, content is much more important than its package.

Hi,

Does this mean that I can use any format for FMEA and still complying to the 4th Edition requirement?, why that all the format I am seeing is having the OCC after the Current Control (Prevention)?, still having the confusion..
