I can't answer the part about using "any format", but as far as the second question...

The Occ column in after the Preventions column because you are supposed to consider the occurance in light of the preventive actions that you have taken. If you complete more preventive actions, you can re-evaluate the occurance.

The Det column is after the Detections column because you are supposed to consider the likelihood of detecting the problem via the detection actions you are planning to take.

Remember, the AIAG FMEA (4th Ed.) Manual is a set of guidelines, not a requirement. The use of the manual is often required by automotive customers, but the FMEA process and its associated form and layout is determined by you.

HI,

for a particular failure mode or Potential cause consideration, If I am not able to determine the current process control (prevention), can I put 'none' in the column and just consider the OCC as it is or I must fill up the column?. Appreciate some input on this.

Hi rku140,

I've seen some FMEA with empty collumn for current process control (prevention) and according to them, the occurence are totally based on the happening rate of that failure in their system. But I personally feel that we should have something in control such as work instruction or standardize operation sheet that we can put in as a control method.

FMEA 4th Edition severity, occurrence and detectability rankings

Hi guys.
I have to update my pFMEA procedure but I can't wait for the AIAG manual, as I will be able to order it only next month. Does anybody have the severity, occurrence and detectability rankings listed in the FMEA 4th edition?
