Rightly said.

Have seen most people stuck up with forms, thinking they are sacrosanct, holy designs. The forms are means for how to count things, which succeeds, 'why' and 'what' of it.

If current process control is only "P" and no "D" action, how can I assess the "Detection" value? Does it mean the "Detection" value should be 10?:eek:

Interesting question, I am also keen to know what is the definition of Controls Prevention in Current Process. Is it a system where it is able to prevent the Potential Cause from happening...?
If it is so where Controls Prevention is good enough to prevent Potential Cause from taking place then it will no longer be detected or detectable by Controls Detection. Then Detection value will be at the high end..eg. 10??
If that is the case, then even if it has good Controls Prevention system in the current process but the Detection may still has a high value if it is not detectable........
Anyone please enlighten me on this...

For those of you that might be "anal" like me and must have your pFMEA and dFMEA forms look verbatim like the ones in the FMEA 4th Edition blue books I have attached these two excel spreadsheets. The only thing different is that the line for "Core Team" does not go completely across the page. Compare with pages 24 and 74 of the 4th edition FMEA blue book...this is just one of my many little idiosyncrasies.
From the excel sheet format in dFMEA & pFMEA,
For the 'Classification' column,
CC is assigned when Sev > 8, AND Occ > 1,
SC is assigned when Sev > 4, AND Occ >3,

Is this the mandatory requirement mentioned in the 4th edition ?

Also what is the difference between CC & SC ?

CC should mean "critical characteristic", SC "safety characteristic".


About to ask the same. In other hand, I'm developing a dFMEA for a textile process; I checked some examples in the book and I saw that them has a lot of details. e.g. for 1 potential failure mode are 4 or 5 potential causes of failure, this needs to be done like this or I can do it for my needs.

It is not some much for your needs as for your process. A simple process may only have 1-2 potential causes for a particular failure mode, while a more complex process might have 9-10 potential causes spread across multiple operations.

hi, recently I show my FMEA's (D and P) and I got from the auditor the comment that I'm mixing in process with product stuff. Can some one provideme an example (template) of both FMEA's?
