This spreadsheet will do nicely

The spread sheet works well . However some of the audit questions need to be rephrased because the OK condition would be the yes answer, which leads to a reduction of the 5S score. Just one thing to look out for.

i hope the thread starter would not mind. i would like to share my audit sheet that i compiled after surfing similar sites and of course by my experience.

it has got only 1s and 2s. because, i need time to get on to 3s.
