It's not difficult to create a spread sheet once you decide on the system. You can look at this one that I posted a while back.

I would definitely like to use this format as I build a system with my new employer. I notice it is protected and would like to change some terminology to match my SOPs. What is the password for the: File Type: xls Supplier Rating form.xls (25.5 KB, 1269 views)

The OP has that. Try opening a new excel file, copy from Supplier Rating form, paste to the new file and save it with a new file name.

Or.... Unprotect Excel Spreadsheets.
