I found JMDN codes on the PMDA website. Apparently PMDA doesn't mind sharing their codes with the world after all.

I couldn't text search the pdf you attached. PMDA website doesn't show risk classifications, just the code.

Is there a searchable pdf or translated copy that have all the risk classes on them? I would like to have one. Health Canada prepared one that is really super helpful. Canada used FDA Product Code names (which GMDN had also borrowed). Then Canada used GHTF risk rules and dropped in their own adjusted risk classification numbers next to each device. This is the right way to do it. Nice to see Japan doing the same.

There is not a PDF, PMDA website can get the JMDN Code, however, i can't paste the address here, please go to the PMDA website directly, choose "medical device standards", then click "standads list" on the left of the navigation and you can get the JMDN Code of medical devices.
