You need to submit all of the documents required by your customer. This can vary widely. If you are not sure what needs to be submitted, ask the customer. If you've been told to submit a level-3 PPAP, the list of requirements is on 18 of the 4th edition PPAP manual. Keep in mind that the various PPAP submission levels control only what gets submitted; you're expected to complete all of the relevant elements and keep on file the ones that don't get submitted.
The forms in appendices C, D and E are similar but different. "C" is for dimensional results, "D" is for material test results and "E" is for performance test results. You are not necessarily required to use those forms, but what you do use should provide all of the required information.
You should not use anyone else's PPAP as a guide, because, as I said, customer requirements and expectations differ. It's not unusual for customer representatives to not understand their own requirements, or those of the PPAP manual.
